Reviews physician’s orders to ensure they are appropriately recorded resident’s chart, Kardex/Care Plan A, Medication Administration Record (“MAR”), treatment/care plans, are implemented as required.*.Reviews charting in nurses notes for accurate informative and descriptive recording that reflects the care provided as well as the resident’s response to the care.Reviews documents for all pertinent data as required by Medicare, Medicaid, Extendicare standards and policies.Completes the required forms upon admission, transfer, and/or discharge.Informs physician of resident change of condition.  8.      *Ensures controlled drugs are counted by two nurses at shift change and notifies the DON of all drug and narcotic discrepancies. 9.      Completes follow up investigation and as needed provides or arranges for follow up education related to any medication errors.  10.  *Ensures medication room and medication cart are locked according to regulations.   11.  *Follows up on any pharmacy recommendations to assure response and implementation of any order changes.12.  Conducts weekly shift meetings with nursing assistants. 13.  Participates in the Performance Improvement Process. 14.  Participates in the Interdisciplinary Plan of Care (“IPOC”) process for assigned residents. 15.  *Complies with laws and regulations applicable to position and acts in accordance with Extendicare’s Corporate Compliance Program. 16.  Treats all residents, visitors, and staff with courtesy. 17.  *Performs physical activities including standing, lifting (up to 60 pounds unassisted), bending, stooping, pushing, pulling and twisting. 17. *Attends and participates in inservice classes, performance improvement (“PI”) committees, and other meetings as assigned.  18.  Works overtime, holiday and weekend hours as scheduled. 19.  Follow facility dress and hygiene policies.  20.     SafetyKnows and follows facility rules.Demonstrates proper use of equipment.  Reports equipment needs or repairs.Follows facility smoking policies.Reports and documents any incidents or accidents of residents, staff or visitors to the appropriate facility personnel.Reports all hazardous conditions/equipment to Supervisor.*Performs duties, which may include transportation of residents, as assigned in Facility Disaster Plan.Uses required protective equipment.Follows infection control standards, policies and procedures. 21. Resident RightsKnows Resident Rights.  Help the residents/patients exercise and/or protect their rights.Reports resident/patient complaints to management.Maintains confidentiality of resident/patient information. 22. HIPAAFollows and adheres to Extendicare’s policies and procedures implementing HIPAA requirements for the privacy and security of protected health information.Uses and/or discloses only minimum amount of Protected Health Information necessary to complete assigned tasks.
